鼻咽癌组织中 c-IAP2 的表达及其临床意义

Expression of c-IAP2 in Nasopharyngeal Carcinoma and Its Clinical Significance

摘要: 目的 探究细胞凋亡抑制蛋白2 (cellular inhibitor of apoptosis protein 2, c-IAP2) 在鼻咽癌组织中 的表达以及临床意义。 方法 回顾性分析 95 例鼻咽癌患者的肿瘤组织和 40 例正常患者的鼻咽组织及临床 预后资料, 分别利用 RT-PCR、 Western 印迹以及免疫组织化学染色检测上述组织中 c-IAP2 mRNA 和蛋白的 表达水平; 卡方检验探究肿瘤组织中 c-IAP2 表达与临床病理学特征的关系; 采用 Kaplan-Meier 结合 Logrank 检验表达不同 c-IAP2 表达水平患者的生存差异; 采用单因素和多因素 Cox 比例风险回归模型分析影响 鼻咽癌患者预后的危险因素。 结果 c-IAP2 mRNA 及蛋白水平在鼻咽癌组织中显著高于正常组织 ( t = 12. 62, P< 0. 001); 进一步, 免疫组化分析发现 c-IAP2 在鼻咽癌组织呈高表达, 且其高表达与肿瘤 T 分 期、 N 分期、 TNM 分期及分化程度显著相关 (P均< 0. 05); 生存分析显示 c-IAP2 低表达组鼻咽癌患者的总 生存率显著高于高表达组 (χ 2 = 11. 86, P = 0. 001), 且 c-IAP2 高表达是导致鼻咽癌患者总生存期缩短的一 个独立危险因素 (HR = 2. 425, 95 % CI: 1. 256 ~ 4. 994, P = 0. 012)。 结论 鼻咽癌中 c-IAP2 的表达水平 明显上调, 其高表达与患者的不良预后显著相关, 对于评估鼻咽癌患者的预后具有潜在的临床应用价值。

Abstract: Objective To explore the expression and clinical significance of cellular inhibitor of apoptosis protein 2 (c-IAP2) in nasopharyngeal carcinoma (NPC). Methods The NPC tissues and the nasopharyngeal normal tissues were obtained from 95 NPC patients and 40 non-NPC patients, respectively, and their clinico-prognostical data were analyzed. The expression of c-IAP2 mRNA and protein in these tissues was detected by RT-PCR, immunochemistry and Western blotting, respectively. The relationship between the clinicopathological characteristics and c-IAP2 expression was analyzed by the chi-square test. Kaplan-Meier combined with Log-rank was used to test the survival difference of patients with different expression levels of c-IAP2. Moreover, univariate and multivariate Cox proportional risk regression models were used to analyze the risk factors affecting the prognosis of NPC patients. Results RT-PCR showed that the mRNA expression level of cIAP2 was significantly increased in NPC tissues when compared with normal tissues (t =12. 62, P< 0. 001). Western blotting analysis showed that the expression level of c-IAP2 protein was significantly up-regulated in NPC tissues, which was consistent with the change of c-IAP2 mRNA expression level. Furthermore, the immunohistochemical analysis displayed that c-IAP2 was highly expressed in NPC tissues, and its expression level was associated with tumor differentiation, T staging, N staging, and TNM staging (P < 0. 05, respectively). Survival analysis found that the overall survival rate in NPC patients with low c-IAP2 expression level was significantly higher than that in NPC patients with high c-IAP2 expression level (χ 2 = 11. 86, P = 0. 001), and the high expression of cIAP2 was an independent risk factor for shortened overall survival of patients with NPC ( HR = 2. 425, 95% CI: 1. 256 ~ 4. 994, P = 0. 012). Conclusion The expression level of c-IAP2 in NPC is significantly up-regulated, and its high expression is significantly correlated with the poor prognosis of NPC patients, which has potential clinical value for evaluating the prognosis of patients with NPC.
