Abstract: Cerebral ischemia-reperfusion injury (CIRI) occurs when cerebral blood flow is restored following an ischemia episode. Although oxygen and nutrients are depleted during ischemia, the subsequent rapid reperfusion can paradoxically cause irreversible neuronal damage. CIRI involves multiple programmed cell death (PCD) pathways, including apoptosis, pyroptosis, and necroptosis. The recently proposed concept of PANoptosis highlights the close integration and molecular crosstalk among these three PCD modalities in inflammatory injury settings, offering a novel perspective for elucidating cell death mechanisms in CIRI. This review summarizes the definition of PANoptosis, the molecular composition of PANoptosomes, and the potential roles of different PANoptosome subtypes in CIRI, aiming to open new avenues for therapeutic intervention and drug development.
